COVID policy
To ensure the safety of our patients, we have installed state of the art germicidal UV-C lights in all our patient exam rooms. As a way to further sanitize and disinfect these rooms beyond Cavicide cleansing after each patient, these lights kill 99% of all viruses (including coronavirus) bacteria, mold, and spores on exposure.
Both of our Providers and Staff have been vaccinated against COVID 19.
Upon your arrival at the office you will be asked to sign a COVID 19 waiver where you will be asked to affirm the following:
- Neither you nor any member of your household has been diagnosed with COVID 19 in the past 30 days.
- You are not currently experiencing fever, fatigue, shortness of breath, cough, or any other symptoms associated with COVID 19.
- Neither you nor any members of your household have travelled by sea or by air, in the past 14 days.
- Neither you nor any member of your household has visited any area in the USA that was reported to be highly affected by COVID-19 in 14 days.
If you have questions pertaining to these policies, please call before coming into the office.
To combat the spread of COVID 19, our office is using recommended protocols, including:
- Televisits
- Patient and visitor screening
- Masks for all employees and patients
- Modified schedules to reduce the number of patients in the waiting room
- Advanced cleaning between patients
- Use of our patient portal for office forms. Call for the code or ask about access when you make your appointment.
Please help us by arriving at our office at your scheduled appointment time. Patients are asked to have guests wait for them outside of the office unless they are a caregiver or a parent accompanying a child.

Appointments
We are accepting new patients of all ages. Please call our office to schedule an appointment. We now require that new patients complete paperwork prior to your appointment via our Patient Portal.
As a courtesy to our patients, we text to remind you of your appointment three days in advance, but you are still ultimately responsible for remembering the time and date of your office visit. Should you need to cancel or reschedule, please do so with 24 hours’ notice. This allows another patient the opportunity to be seen for urgent matters. New patients arriving more than 5 minutes late and existing patients arriving more than 10 minutes late may be asked to reschedule. Please update all paperwork on the Patient Portal prior to your appointment, particularly if it has been a year or more since your last appointment.
Televisits are being scheduled for your convenience. These appointments are an excellent alternative to in-person follow-up visits for our Accutane patients, students, and others who require frequent follow-ups.
Surgical and Cosmetic Filler Appointments
Due to the amount of time needed for surgeries and cosmetic fillers, Dr. Ha requires multiple confirmations for these appointments. You will receive a text from our automated system three days before your appointment.
You will also receive a call from one of our staff members two days before your scheduled appointment. Dr. Ha insists on a “live” verbal confirmation from the patient. If our phones are off, we will accept a message confirming the appointment. If we do not hear from you by noon the day before your surgery/filler appointment, it will automatically be canceled, and you will be asked to reschedule.
Cancellations/Missed Appointments
We appreciate timely notice when an appointment must be canceled or rescheduled. Cancellations and rescheduling with less than 24 hours’ notice will be assessed a $50 fee. Surgical excisions will be assessed a $100. Cosmetic appointments missed or canceled with less than 24 hours’ notice will be assessed a $150 fee.
Refills
Should you need a refill of your medication, please request one from your pharmacy. Most prescriptions written by our physicians have refills. Your pharmacy will keep track of these. They will usually contact us via fax to seek authorization if necessary. Requests are usually addressed within 24 hours – often the same day. If your medications require prior authorization from your insurance company, more time must be allowed. Please do not wait until the last minute to order a refill.
If you are overdue for follow-up, you may be asked to schedule an appointment for an examination to adjust your medications or to try something new.
Insurance
Please bring your insurance card with you to every appointment. It is your responsibility to request a referral if needed and bring it with you or have it faxed to our office prior to your appointment. If we do not have a referral when you come in, you may need to reschedule.
